Reminder for Chrysler BCBS PPO Members: In December you were sent, " Your welcome kit for The Chrysler National PPO", it included requirements to retain a lower deductible and lower out-of-pocket maximums, steps 1 and 2 are due by March 31, 2011.
Steps 1 and 2 include an annual physical, physician completed Qualification Form and Online Health Risk Assessment.
Steps 3 and 4 are due by May 31, 2011.
